Title:                     Community Outreach Nurse
Wage:                  $18.25 per hour
Work Location:  Pittsfield, MA with travel in Western Massachusetts
Hours:                  10-15 hours/week, flexible including some weekends/evenings

Employee Profile:
The Community Outreach Nurse is responsible for overall management of the medical aspects of the AFC and MassHealth programs including the direct oversight and supervision of staff. The nurse in responsible for administration of the medical/nursing components of the AFC program.   S/he is responsible for ensuring programmatic and contractual obligations are met. The ideal candidate must possess organizational and creative abilities; ability to work independently in a team environment and be flexible to meet program needs.   S/he implements the organization’s mission and values by supporting individuals to make positive changes in their lives.  Must have valid driver’s license, good driving record, use of vehicle, and willingness to transport program participants in personal vehicle. The desire to do meaningful work and make a difference in someone’s life a must. R.N with Bachelor’s degree in Nursing or Human Services preferred or R.N. and a minimum of three years experience or RN with five years experience. Bilingual/bicultural experience an asset.
